The Department of Social and Health Services (DSHS) levied civil fines totaling $11,000 on Evergreen Bremerton Health and Rehabilitation Center, a privately operated nursing home, for violating state and/or federal standards of care.

DSHS imposed the fine for deficiencies related to prevention of abuse/neglect, quality of care/activities of daily living, quality of care/skin, and administration that were identified during a full standard survey recently completed at the nursing home.

The 62-bed nursing home is located at 3517 11th St. in Bremerton. It is licensed to and operated by Martini Investments Inc. and serves Medicaid, Medicare, and private-pay residents.

State law requires nursing homes to make complete copies of the DSHS Statement of Deficiencies and the nursing home’s Plan of Correction available for public viewing whenever violations occur.
